Registering with us

We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by bringing along your NHS medical card or completing a form available from reception. We will need to see evidence of identity e.g. Photo I.D. (passport, driving licence, official identity card) or Birth Certificate or Marriage Certificate. Evidence of address will also be needed e.g. Council tax bill, paid utility bill, wage slip, bank statement. The registration forms can be downloaded from our website for your completion before coming into the practice to register.
All new patients are offered a check-up on joining the practice. This is very important for us to manage patient care until full medical records arrive.

Changing your address or name

Please notify the surgery of any change of address or name in order that medical records can be updated, and do remember to include your new telephone number and postcode.

The main premises are at Crick with a branch surgery at West Haddon.

We are a teaching practice which means from time to time students from Warwick Medical School do sit in with the doctors. Please inform the receptionist if you do not wish students to be in attendance.
